The Fetal Medicine provides care for pregnant women by doctors who have specially trained in ultrasound scanning , councelling and invasive procedures . In fetal medicine detailed sonography is performed to the highest standards and is backed up with informative and sensitive counselling as well as with prenatal diagnostic and therapeutic procedures (Fetal Therapy).
PROTOCOL for PREGNANCY at PULSE WOMEN'S HOSPITAL:
Fetal Viability Scan 6-10 weeks
This is an ultrasound examination that is usually carried out vaginally. It aims to determine the number of fetuses present and whether the pregnancy is progressing normally inside the uterus [ and not an ectopic pregnancy ]. This scan is useful for women who are experiencing pain or bleeding in the pregnancy and those who have had previous miscarriages.
Nuchal Translucency and First Trimester Screening- [ 11 & 14 weeks ]
We perform the First Trimester Down's Syndrome Screening according to the standards laid down by the Fetal Medicine Foundation, UK. This screening involves an ultrasound assessment of the fetus including the nuchal translucency, and the new markers such as tricuspid regurgitation, ductus venosus, nasal bone and facial angle assessment.
The maternal blood is also checked for PAPP-A and free beta- HCG and an integrated risk assessment for the chromosomal abnormality is given. FTS is highly accurate and its sensitivity to identify fetus with Down`s Syndrome is 85% to 90%.
Anomaly Scan 18-22 weeks
This is a detailed scan during which each part of the fetal anatomy is examined. Special attention is paid to the brain, face, spine, heart, stomach, bowel, kidneys and limbs.
If any abnormalities are detected the significance of the findings will be discussed and the couple will be given the opportunity to have further counseling with a Fetal Medicine Consultant.
a. Cervical Screening 18-24 weeks
This is a transvaginal scan done to assess the women`s risk of going into preterm labour and delivery at less than 33 weeks by measuring the cervical length.
b. Colour Doppler
The uterine artery dopplers are checked to assess the woman's risk of developing Pre-eclampsia or intra-uterine growth restriction. The Umbilical artery , MCA ,Ductus Venosus , Fetal Aorta and other arterial and venous dopplers are performed in cases of Pre-eclampsia , intra-uterine growth restriction or other high risk pregnancies.
c. Fetal Echocardiography
A detailed examination of the fetal heart and connecting vessels is carried out usually at 20 weeks by a consultant Fetal Medicine specialist. It is especially recommended for people with a previous pregnancy history or family history of heart abnormalities, or in diabetic mother or where increased nuchal translucency has been found at the 12 week scan. It is also an important ultrasound scan when other abnormalities have been found in the fetus.
Fetal Well Being Scans 24-40 weeks
We advise an ultrasound scan to assess fetal well being and fetal growth to all women at about 30-32 week of pregnancy. This scan is especially important for those women who have had previous complications of pregnancy such as pre-eclampsia, growth retardation, diabetics, stillbirth and for those women who develop an abnormality during the course of their current pregnancy.
3D - 4D Scans
This is a scan during which fetal anatomy is examined in 3D - real time 4D. This scan is especially important for those women who have had previous complications and for those women who develop an abnormality during the course of their current pregnancy.
The clear view of the fetus from all angles allows doctors to detect any congenital abnormalities at an early stage and chart a course for corrective measures at an early and preventable stage. The image quality is so clear and sharp that one can get a fairly accurate impression of how the baby's features will look upon birth.
One can view the baby yawning, sucking its thumb, kicking its feet, and moving its hands. We also provide all pregnancy sonography patients with a collection of video clips and images of the unborn child to create a lifelong memory for the mother and the child!
